Role Overview
As a Community Support Worker, you will meet with individuals, families and carers to connect and listen and where necessary undertake an assessment under the Care Act 2014. Where there is an eligible need, you will devise a plan for how the person can access the support they require – this may be connecting with their local community, referral onto other agencies or providing service to support their unmet need. Where necessary, you will identify and report potential safeguarding, consider mental capacity, and other high‑risk situations where a person's safety or liberty may be at risk. Your team will have an office base and can work in a hybrid way. You will undertake visits to people in their own homes, in a hospital setting or wherever the person feels most comfortable to meet you.
We are seeking to appoint a Community Support Worker who wishes to join our Mental Health Team serving the Blaby, Oadby & Wigston, and Harborough areas. If successful, you will be part of a close‑knit, collaborative service dedicated to supporting people to live well in their communities. You will work alongside experienced Community Support Workers, Community Reablement Workers and Social Workers, based in Bassett Street in Wigston, contributing to a multi‑disciplinary approach that values shared learning, mutual support, and a strong team ethos. Together, we provide person‑centred support that empowers individuals to build resilience, develop independent living skills, and access the right services at the right time. If you are passionate about teamwork, community‑based mental health support, and making a positive difference across diverse localities, we would be keen to hear from you.
