Overview
We are seeking a Band 6 Community Learning Disability Nurse to join the Gwynedd Learning Disability Community Health team at Betsi Cadwaladr University Health Board.
Responsibilities
* Work as part of the Learning Disability team at Gwynedd, ensuring interfaces between the Local Authority, neighbouring services, community mental health teams, inpatient services, statutory agencies and the voluntary sector.
* Deliver evidence‑based, safe and effective interventions and clinical care following the nursing process, supporting service principles and strategy for people with learning disabilities and meeting agreed standards.
* Provide a visible and accessible member of the team, making information and support available to patients and their families.
* Communicate and receive highly complex and sensitive information, analysing and interpreting data and acting appropriately where barriers to acceptance may exist.
* Apply a good understanding of the CHC process and contribute to the development of pathways and the Maturity Matrix.
Qualifications
* Band 6 nurse, registered with the Nursing and Midwifery Council.
* Experience in learning disability/community nursing or equivalent.
* Strong communication skills and the ability to work collaboratively across services.
* Analytical skills to handle complex information.
* Knowledge of the CHC process and pathway development.
* Desirable: Ability to speak Welsh; Welsh and/or English applicants are welcome.
