Overview

The Autism Post‑Diagnostic Support Worker will provide person‑centred, strengths‑based support to autistic adults following diagnosis, helping them build skills, confidence and self‑esteem to live fulfilling, meaningful lives in their communities. The role works alongside individuals in one‑to‑one and group settings, collaborating with families, carers and partner organisations.


Key Responsibilities

* Support autistic people to build practical skills for daily living, including routines, planning, organization and self‑care.
* Promote self‑esteem, self‑acceptance and a positive autistic identity.
* Assist service users in developing, accessing and maintaining meaningful social, community, educational or vocational roles.
* Enable individuals to identify strengths, interests and personal goals, focusing on abilities rather than difficulties.
* Support users to access and navigate local services, networks and community resources.
* Encourage autonomy, self‑advocacy and informed decision‑making.
* Work in partnership with families, carers and other agencies to provide consistent and coordinated care.
* Contribute to a strengths‑based, inclusive culture that values neuro‑diversity and lived experience.
* Act as a positive role model, demonstrating organisational values while engaging with service users and colleagues.
* Establish supportive and respectful professional relationships with service users.
* Utilise recovery tools, techniques and strategies to help users identify strengths, interests and goals.
* Share and promote coping, self‑help and self‑management techniques alongside neuro‑affirming discussions.
* Liaise with carers, health professionals, statutory and voluntary agencies to support users' needs.
* Accompany service users to appointments or meetings and perform practical tasks aligned to well‑being goals, promoting choice.
* Deliver interventions in homes, community settings and groups, tailoring approaches to individual needs.
* Facilitate access to community groups and networks, enabling participation in socially valued roles.
* Advocate for service users and assist them to gain access to benefits, training, education, employment and housing.
* Integrate into a multi‑disciplinary team, using initiative, personal experience and training to decide on appropriate interventions.
* Support development and facilitation of therapeutic group work.
* Maintain accurate patient health records in line with national and organisational policies.
* Align support work goals with the Trust’s record‑keeping process and review regularly with the MDT and Autism Practitioners.
* Participate in team check‑in sessions, MDT meetings and clinical records to ensure continuity of care and user safety.
* Engage in regular line management, clinical supervision and reflective group supervision to identify personal development needs.
* Raise awareness of neuro‑affirming language, modelling non‑discriminatory, trauma‑informed communication.
* Promote a recovery or neuro‑affirming oriented environment, facilitating shared education and activity planning.
* Contribute to quality assurance measures, including uptake of satisfaction questionnaires and other tools to review service effectiveness.
* Motivate and encourage users and carers to participate in the Trust Service User and Carer Engagement Scheme and Volunteering Scheme.
* Attend clinical meetings to provide feedback on recovery goals.
* Actively engage in training to support competency for the role.
* Deliver awareness training for colleagues and external agencies.
