The Bellfield Centre is looking for an experienced, dynamic and dedicated Intermediate Care Worker to join our vibrant and supportive team. This is an exciting opportunity for someone with a background in social care who is passionate about making a real and lasting difference to the lives of older people using our integrated Intermediate Care service.
About the Role
As an Intermediate Care Assistant, you will play a key role in supporting older people to regain independence, improve wellbeing and achieve their personal outcomes. You will work as part of a fully integrated health and social care team, providing high‑quality, person‑centred care and support.
We Are Looking For Someone Who:
1. Has experience working as a Social Care Assistant or in a similar care role
2. Is passionate about improving outcomes for older people
3. Demonstrates strong values, compassion and a person‑centred approach
4. Understands Intermediate Care and its role in supporting recovery and independence
5. Has excellent communication and organisational skills
6. Enjoys working collaboratively as part of a committed multidisciplinary team
Essential Requirements:
7. SVQ Level 2 in Social Care
8. Experience delivering professional care and support to older adults
9. Ability to work effectively within a busy and evolving care environment
10. Candidate's should be registered with the SSSC or in the process of registering
