The St James Adult Medicine Team is looking to recruit a motivated rotational specialist clinical pharmacist. We have a variety of rotations including Older Peoples Care, Respiratory and CF, General Medicine, Acute Medicine, ED, and SDEC.

Main duties of the job
To provide pharmaceutical care to patients within a clinical area. To cover for other pharmacists within the Clinical Pharmacy Team as needed. To support the Lead Pharmacist by assisting in the development of drug-related policies, procedures, financial reports, and horizon scanning activities.

Job responsibilities
Provide specialist clinical pharmacy services to patients and the multidisciplinary team, being professionally accountable. Offer drug-related advice to patients and healthcare professionals. Monitor drug use, advise on optimization, report near-misses, and support risk management. Educate healthcare professionals and train junior pharmacy staff. Assist in developing policies, procedures, and financial reports. Participate in operational services, including dispensaries and Medicines Information. Work evenings, weekends, and bank holidays on a rotational basis, including on-call duties. Rotate to different specialties every 6-12 months.

Qualifications
* GPhC registration as a Pharmacist
* Postgraduate Diploma in Clinical Pharmacy or equivalent
* MPharm
Experience
* Broad clinical pharmacy knowledge
* Hospital pharmacy practice experience
