We are recruiting a practice nurse to replace a leaver. The successful candidate should have an interest in diabetes as they will have responsibility for diabetes disease management and the annual reviews and QoF. Also assist in chronic disease management clinics. Salary dependant upon experience.

About us
We are a large, busy town centre practice with 16,700 patients. The team currently consists of 8 GPs, 2 ANPs, 3 nurses, 1 HCAs and a pharmacist. The reception and admin function currently has 17 staff. We are part of the Synergy PCN delivering services to a large part of Uxbridge. We have the support of PCN staff in the form of a pharmacist, pharmacy technicians, physiotherapist, social prescriber, mental health nurse and community matron with CCT team.
Job responsibilities
REPORTS TO: The Lead Nurse (Clinically), The Practice Manager (Administratively)
GENERAL PURPOSE: To undertake daily clinics as arranged for the treatment of registered patients. To include health promotion as necessary. All clinics are open to variation as necessary according to current legislation and/or patient care. Practice policy states all protocols and guidelines available are to be followed. Duties include: ECG tracings, immunizations and vaccinations (adults and children), including seasonal flu campaign, intramuscular, intradermal and subcutaneous injections as authorized by the patient's GP, knowledge and application of emergency procedures for anaphylactic shock, assisting with minor operations, cervical smears in line with national screening policy, maintaining the computerized patients' clinical database and manual medical records with contemporaneous notes, conducting new patient health checks, NHS health checks, attending weekly PHCT meetings or at least one per month, wound care as referred by GPs or colleagues, tissue viability (wound and ulcer care), contraception injections and routine checks, and any other duties as may be reasonably required by employer. Appropriate referral to other agencies when appropriate.
