The Role
The main role the Care Experienced Young Persons Nurse - Throughcare and Aftercare is to provide caring and compassionate interventions that are underpinned by Scotlands Promise to young people who are identified as needing additional support as they move towards adulthood. The foundations of the Promise will facilitate the role of these nurses to provide a scaffold of support to young people who are linked with North Lanarkshires Through and Aftercare Teams.
Care will be specifically focussed on health interventions that can maximise improvements in short and longer term outcomes for the young people identified. These interventions will be delivered as part of a whole system compassionate response to what the young person identifies that they need to grow and develop into healthy adulthood.
The working pattern for this role is Monday - Friday; 9:00-5:00.
In this key role, you will:
To undertake complex assessment, plan, implement and evaluate individual, comprehensive health assessments and action plans, ensuring in partnership with care experienced people, are appropriately involved in decision-making about their care using motivational interviewing skills, scaffolding and promoting self-efficacy.
Understand and demonstrate the application of trauma informed practice, teenage brain and how a strength/asset based approach can support young people in this area.
Understanding of health inequalities and tackling poverty, working with colleagues and partners through interpretation of the local data and determinants
Work collaboratively with colleagues in through care and after care team, parents, Health, Social Work, and carers to support pathway plans, to ensure that the health needs of care experienced people and Care Leavers are addressed through multi agency care planning and joint working.
To work with all professionals involved, as the expert nursing practitioner, with the young person and their families to assess, analyse, develop and agree a multiagency plan based on the agreed policies and procedures. Implement and monitor the plan to endeavour to ensure that individual needs are met and desired outcomes achieved.
What You'll Bring:
Registration requirements
Registered nurse at degree level or equivalent
Post-registration qualification in public protection / child protection/Sexual Health/School Health or Health Visiting
Qualifications & Knowledge
Educated to Masters level in a relevant nursing area, or
Can demonstrate an equivalent level of knowledge gained through, for example, completion of NES modules on Assessment, Leadership, and Critical Thinking.
Can provide advice using significant understanding and application of legislation relating to information sharing, data storage and public protection
Knowledge of health and social care pathways related to safeguarding, early intervention and protection
Knowledge of current legislation underpinning practice eg UNCRC, Getting it Right for Every Child and The promise
A driving licence is required; this must be a full UK/EU/EEA licence.
From 1 April 2026, the Agenda for Change full-time working week will reduce from 37 hours to 36 hours. Part-time working hours will be reduced on a pro-rata basis. A corresponding increase in the hourly rate will apply, ensuring that overall pay remains unchanged.
