Job Responsibilities
Provide social work services across the Stoke-on-Trent area, including hospital and community settings.
Undertake assessments, care planning, and interventions in line with statutory requirements and best practice.
Ensure safeguarding of adults and promote their safety, wellbeing, and independence.
Apply relevant legislation, guidance, and social work standards to all practice decisions.
Maintain accurate, up-to-date case records and documentation.
Work collaboratively with families, service users, and multi-agency partners to deliver integrated care.
Contribute to service development, continuous improvement, and identify opportunities for better ways of working.
Travel across the city as required to meet the needs of service users.
Person Specifications
Must-Have
Qualified Social Worker with current or in-process HCPC registration.
Recognised social work qualification.
Minimum of 3 years post-qualifying experience in adult social care.
Detailed knowledge of adult safeguarding principles and best practice.
Understanding of key legislation and guidance underpinning work with adults.
Excellent verbal, written, and analytical skills.
Ability to work across the geographic area, with a valid driving licence or access to alternative transport.
Commitment to continuous improvement and a proactive approach to practice.
Eligibility to work in the UK and valid NI number.
Nice-to-Have
Experience working in hospital or community-based adult social care services.
Familiarity with Stoke-on-Trent City Council procedures and local networks.
Experience contributing to audits, quality improvement, or service transformation initiatives.
Ability to mentor or support junior social workers.
