The Rosemary Foundation is seeking an enthusiastic, committed, and motivated Bank Health Care Support Worker to join our friendly hospice at home team.

The Rosemary Foundation is a 24/7 nurse-led service providing palliative and end-of-life nursing care to adults with a life-limiting illness in their own homes, within a 15-mile radius of our base in Petersfield.

Candidates will need to be able to drive, have a current UK license, and access to a car. Experience in the care environment, confidence to work independently, and flexibility with working patterns including night on call are required.

Candidates should be passionate about delivering high-quality palliative and end-of-life care to patients and their families.

Main duties of the job

To work under the direction of a registered nurse, both independently and as part of the team. Support the nursing team in delivering direct patient care, aiming to provide high-quality, holistic, palliative, and end-of-life care to patients and their families.

You will actively engage with colleagues, patients, families, carers, and volunteers.

Anticipate and respond to changing patient needs while maintaining their independence, privacy, and dignity.

Understand and promote infection control practices and policies.

Assist in the implementation and delivery of patient care under supervision and direction of a registered nurse.

Ensure patients' rights are adhered to and that care choices are consented to.

Follow the patient's care plan, observe their condition, and report observations or changes to a registered nurse.

Provide support and information to patients, families, carers, and colleagues as appropriate and within your role, skills, and knowledge.

Be responsible and accountable for the standard of care given.

Follow guidelines for safe systems at work and maintain a safe working environment.


About us

The Rosemary Foundation is a nurse-led hospice at home team covering 700 square miles in East Hampshire.

We are a charitable organization, and all care and services are provided free of charge. The Foundation is funded by donations, community fundraising, and legacies.

We work closely with GPs, community nursing teams, and specialist palliative care teams to ensure high standards of end-of-life care.


Job responsibilities

Learning and Development

Commit to annual staff appraisals and regular performance and competency reviews for personal development.

Participate in educational programs, including mandatory training and development activities.

Engage in reflective practice and assessment of professional competencies.

Complete the Care Certificate program via e-learning within required timescales.

Undertake all statutory and mandatory training as required.

Professional

Maintain the values, vision, and aims of The Rosemary Foundation.

Practice and promote patient confidentiality.

Maintain accurate and timely documentation.

Practice safely within policies, procedures, and guidelines.

Demonstrate efficient time management and a flexible attitude to working patterns.

Participate in the off-duty rota, working flexibly to provide a 24/7 service.

Work within your competency limits and refer outside areas to a registered nurse.

Participate in training and updates as required.

Treat everyone with respect and dignity.

Communication

Communicate effectively within your team and the wider multidisciplinary team regarding patient and family care needs.

Communicate respectfully and confidentially with colleagues.

Alert the nurse in charge to urgent or sensitive matters and escalate as appropriate.

General Responsibilities

Uphold the values, vision, and aims of The Rosemary Foundation.

Undertake additional duties as discussed with your line manager.

Represent the Foundation in the community and on social media appropriately.

Adhere to all policies and procedures, including safeguarding policies.

Ensure personal safety and report health and safety concerns.

This job description may be amended according to service needs, in consultation with the post holder.

Flexibly undertake other duties consistent with your role, including project work and cover for absences.

Qualifications

* Experience in a healthcare environment

* NVQ Level 2
* Care Certificate
