We are looking for a full time or part time Staff Nurse.
We are looking for nurses with exceptional communication and interpersonal skills to join our team. You will work closely with the Clinical Leads to ensure more people achieve a peaceful and comfortable death in their preferred place of care.
We offer a flexible approach to rostering and we will consider part-time applications; early, late, and twilight shifts are available. However, please note that night shifts are currently a mandatory requirement of the position (usually 2-3 per month pro rata).
Main duties of the job
* Understand the physiological, psycho-social, spiritual, and cultural needs of patients requiring specialist palliative care and their families/carers.
* Complete the first level assessment of the patient's needs and document these on the care plan.
* Using the information obtained from nursing history assessments and from subsequent observations, identify the patient's individual nursing problems and needs, and plan and implement care.
* Set goals for the resolution of each of the identified nursing problems, and help the patient achieve these goals through appropriate care planning.
* Document and evaluate care given and discuss progress, condition, and treatment with the multi-disciplinary team, alerting the Team Leader and/or Senior Staff Nurse of the patient's changing health care needs.
* Assist with teaching patients and their family/carers in preparation for changes in treatment and expectations, and discharge for home care.
* Assist more senior members of the nursing staff in the organisation and management of the clinical area ensuring that the principles of individualised nursing care are maintained.

Job responsibilities
