Lead seamless care transitions between hospital walls and community settings in regional Tamworth. Where you’ll be working Join the Hospital in the Home (HiTH) service at Tamworth Hospital, delivering acute care as part of Integrated Care Services. Conveniently located in the heart of Tamworth – a vibrant regional city with friendly country charm, easy access and good parking – you'll lead a warm, collaborative team connected to medical and allied health partners for seamless patient transitions and strong outcomes. About the role As Nurse Unit Manager (NUM), you'll oversee day-to-day delivery of high-quality, patient-centred acute care in home settings, working collaboratively with medical and allied health teams to ensure coordinated care and equitable access. You'll mentor and guide the nursing team, drive service excellence through safety, compassion and continuous improvement, and contribute to a solutions-focused culture prioritising the right care in the right place. You'll feel confident from day one with comprehensive orientation via the Excellence Framework, tailored education, monthly accountability meetings with the operational manager, and 30- and 90-day check-ins for ongoing professional development and support to shape this dynamic service. About you You're a steady, caring leader who brings clarity and trust to dynamic community care environments, with a passion for acute/complex cases and mentoring staff through change. You bring: Current AHPRA registration as Registered Nurse (RN) Relevant management qualification or equivalent experience Strong communication and relationship-building skills Demonstrated clinical leadership and staff mentoring capability Collaborative, proactive approach to safety, quality and innovation Current NSW driver's licence for community-based service coordination Interest/background in HiTH or community acute care is highly desirable and can be developed in the role.
