In your role, contribute to supporting statutory Care Act Assessments for Older People and clients with Physical Disabilities. The goal is to meet 28-day performance targets and reduce waiting times for new referrals. Deal with complex long-term cases, including hoarding, non-engagers, DoLS challenges, and court cases related to welfare disputes. Responsibilities include conducting six-week reviews, performing Mental Capacity Assessments, completing Continuing Healthcare assessments, and advancing Safeguarding cases exclusively assigned to Social Workers within the team.
