Main duties of the job The successful candidate will provide high-quality, patient-centred general medical services including face-to-face and telephone consultations, management of acute and chronic conditions, prescribing, and clinical administration associated with patient care.

The role involves working collaboratively within our multidisciplinary team to deliver safe, effective, and compassionate care to our diverse patient population. The postholder will also contribute to clinical meetings, quality improvement initiatives, and continuity of care within the practice.

Our multidisciplinary team consists of Salaried GPs, Pharmacists, Nurses, Physician Associates, HCAs, PCN ARRS staff, a Practice Manager, Reception Manager, Medical Secretary, Administrative team, and Reception staff, all working together to provide high-quality patient-centred care.

Job responsibilities Job Title: Salaried GP with Special Interest in DiabetesLocation: Brentford Family Practice, Hounslow, West LondonSessions: four Sessions per week

Job Summary The postholder will provide high-quality, patient-centred primary care services to the practice population while working collaboratively within a multidisciplinary team. The successful candidate will support the delivery of safe, effective, and compassionate healthcare services and contribute to the ongoing development of the practice, with a particular interest in diabetes management and chronic disease care.

Main Duties and Responsibilities Undertake face-to-face, telephone, and online consultations for patients presenting with acute and chronic conditions Provide clinical assessment, diagnosis, treatment, and management plans in line with best practice guidelines Lead and support the management of patients with diabetes and other long-term conditions within the practice Work closely with the multidisciplinary team to improve diabetes care outcomes and patient education Support chronic disease management initiatives and contribute to quality improvement in diabetes care Prescribe medications appropriately and safely within current regulations and practice policies Process clinical correspondence, pathology results, prescriptions, referrals, and other associated administrative tasks Maintain accurate and contemporaneous patient records using the clinical system Work collaboratively with the wider multidisciplinary team including pharmacists, nurses, physician associates, HCAs, and PCN ARRS staff Participate in clinical meetings, practice development, audits, and quality improvement initiatives Contribute to safeguarding, significant event reviews, and clinical governance activities Maintain professional registration and continue professional development in line with GMC and appraisal requirements Adhere to all practice policies, procedures, and confidentiality requirements

Person Specification

Fully qualified GP with GMC registration and licence to practise

Included on the NHS England Performers List

Excellent communication and interpersonal skills

Ability to work effectively within a multidisciplinary team

Commitment to delivering high-quality patient care

Good organisational and time management skills

Strong interest and experience in diabetes management and chronic disease care

Commitment to continuous learning and professional development

Diploma or additional qualification in Diabetes Care

Experience leading or supporting diabetes clinics within primary care

Experience working within UK General Practice

Experience with quality improvement or clinical audits

Qualifications

MBBS or equivalent medical qualification

Full GMC registration with licence to practise

Included on the NHS England Performers List

MRCGP qualification or equivalent

Evidence of continued professional development (CPD)

Strong knowledge and experience in diabetes management and chronic disease care

Experience working within UK General Practice

Excellent clinical assessment and decision-making skills

Ability to work effectively within a multidisciplinary team

Excellent communication and interpersonal skills

Good organisational and time management skills

Commitment to delivering high‑quality patient‑centred care

Understanding of clinical governance, safeguarding, and quality improvement

Interest or additional qualification in Diabetes Care

Experience leading or supporting diabetes clinics within primary care

Experience with clinical audits and quality improvement projects

Experience supervising or supporting multidisciplinary team members

Experience working as a GP within UK General Practice

Experience managing patients with acute and chronic medical conditions

Experience in diabetes management and long-term condition care

Experience working within a multidisciplinary healthcare team

Experience using clinical systems and maintaining accurate patient records

Experience undertaking clinical administration including referrals, prescriptions, and pathology management

Experience delivering patient-centred care in a busy primary care environment

Experience participating in clinical governance and safeguarding processes

Experience leading or supporting diabetes clinics within primary care

Experience with QOF and chronic disease management programmes

Experience participating in audits and quality improvement initiatives

Experience supervising, mentoring, or supporting junior staff or trainees

Experience working within PCN or integrated care services

Experience contributing to service development or innovation within General Practice
