Main duties of the job The successful candidate will provide high-quality, patient-centred general medical services including face-to-face and telephone consultations, management of acute and chronic conditions, prescribing, and clinical administration associated with patient care.
The role involves working collaboratively within our multidisciplinary team to deliver safe, effective, and compassionate care to our diverse patient population. The postholder will also contribute to clinical meetings, quality improvement initiatives, and continuity of care within the practice.

Job Summary The postholder will provide high-quality, patient-centred primary care services to the practice population while working collaboratively within a multidisciplinary team. The successful candidate will support the delivery of safe, effective, and compassionate healthcare services and contribute to the ongoing development of the practice, with a particular interest in diabetes management and chronic disease care.
Main Duties and Responsibilities Undertake face-to-face, telephone, and online consultations for patients presenting with acute and chronic conditions Provide clinical assessment, diagnosis, treatment, and management plans in line with best practice guidelines Lead and support the management of patients with diabetes and other long-term conditions within the practice Work closely with the multidisciplinary team to improve diabetes care outcomes and patient education Support chronic disease management initiatives and contribute to quality improvement in diabetes care Prescribe medications appropriately and safely within current regulations and practice policies Process clinical correspondence, pathology results, prescriptions, referrals, and other associated administrative tasks Maintain accurate and contemporaneous patient records using the clinical system Work collaboratively with the wider multidisciplinary team including pharmacists, nurses, physician associates, HCAs, and PCN ARRS staff Participate in clinical meetings, practice development, audits, and quality improvement initiatives Contribute to safeguarding, significant event reviews, and clinical governance activities Maintain professional registration and continue professional development in line with GMC and appraisal requirements Adhere to all practice policies, procedures, and confidentiality requirements

Fully qualified GP with GMC registration and licence to practise
Included on the NHS England Performers List
Excellent communication and interpersonal skills
Ability to work effectively within a multidisciplinary team

Diploma or additional qualification in Diabetes Care
Experience leading or supporting diabetes clinics within primary care

Qualifications
MBBS or equivalent medical qualification
Full GMC registration with licence to practise
Included on the NHS England Performers List
MRCGP qualification or equivalent
Evidence of continued professional development (CPD)
