Out of Hours Care Co‑ordinator
Looking for a new challenge that enables you to work from home. Must live within a 20‑minute drive of Hull.
We are excited to offer an opportunity to join our team. You will be joining a privately owned organisation that offers a range of care services to older people and children within their own homes. Passion for quality care is cascaded throughout the organisation. You will be mentored by a dedicated management team who will help you succeed and develop.
Experience in an on‑call role is not essential as full training will be given. However, at least 2 years of care experience is essential.
If you are passionate about the delivery of high standards of care and support in people’s own homes, have experience in a social care setting, are able to prioritise and solve problems using your own initiative, you may be the person we are looking for.
Responsibilities
* Ensure the safe and effective delivery of care and support to all service users outside of office hours.
* Work evenings and alternate weekends, bank holidays on a rota basis.
* Maintain positive relationships with service users, families and other professionals.
* Receive out‑of‑hours calls from service users, families and colleagues and respond appropriately.
* Resolve issues that may arise and collate accurate notes, taking action in line with the organisation's policies and procedures.
* Deliver care and support to service users at short notice to meet the demands of the service.
* Provide accurate information during daily hand‑overs to ensure a seamless service.
* Monitor the call alert system to ensure all service users have received their care call and staff have arrived safely and on time.

Requirements
* Must hold a full UK driving licence.
* Must hold NVQ level 2 or above in Health and Social Care and be willing to work towards a Diploma Level 3 in Health and Social Care.
* At least 2 years of care experience.
