An exciting opportunity has arisen for Band 7 ECP’s to join the acute out‑patient parental antimicrobial therapy service as part of UHD’s Virtual Ward/Hospital at Home Project.
The post requires flexible working between the acute sites and eventually patients’ homes to administer intravenous antimicrobials. Providing a high‑quality service that is patient‑centred is important to us; the successful candidate will be a highly motivated and enthusiastic nurse with a flexible, dynamic approach to patient care.
Responsibilities
* Screen patients onto the OPAT pathway and assess suitability for IV antimicrobial therapy.
* Manage own clinical caseload of patients, overseeing the delivery of IV antibiotics and ensuring timely assessment and recruitment of suitable patients.
* Coordinate and organise the Virtual Ward/Hospital at Home service across acute sites and home visits for patient care.
* Engage with the lead Consultant Microbiologist and clinical teams from various specialties and community teams, including GPs.
* Work alongside the Treatment and Investigation Unit (TIU) team during initial service delivery.
* Support service improvement, governance and the continued development and expansion of patient pathways.
* Deliver high‑quality, effective, integrated and compassionate care, including admission avoidance and early supported discharge.
