Were looking for a passionate and dynamic Clinical Mobilisation Lead to join a leading complex care provider, supporting adults and children with long-term health needs across the Oxfordshire and Warwickshire region. This is an exciting opportunity for an experienced nurse to take on a pivotal role in shaping outstanding community-based care.
As a Clinical Mobilisation Lead, youll take ownership of setting up new complex care packages, ensuring smooth transitions from hospital to home and delivering the highest standards of clinical governance. Youll provide oversight, mentoring, and guidance to care teams while ensuring that every package is safe, compliant, and person-centred.
Key Responsibilities:
* Lead and coordinate the mobilisation of new complex care packages across the community.
* Conduct clinical assessments and develop detailed, person-centred care plans.
* Deliver clinical training, competency sign-offs, and mentoring for care teams.
* Oversee governance, safeguarding, and CQC compliance during mobilisation.
* Liaise with families, ICBs, CHC teams, and multidisciplinary professionals to ensure continuity of care.
About You:
* NMC-registered nurse (RGN or RNLD) with experience in complex community care.
* Excellent organisational and leadership skills with a proactive, flexible approach.
* Confidence managing clinical risk, governance, and team supervision.
* Full UK driving licence and willingness to travel across Oxfordshire and Warwickshire.
