Job Role
Band 5 Community Staff Nurse – Lower limb wound Care Team
37.5 hours per week
Responsibilities
* Delivering high-quality nursing care to patients in clinics and home environments.
* Undertaking holistic assessments of patients with Lower Limb wounds.
* Using Doppler ultrasound techniques and clinical interpretation skills to support diagnosis and care planning.
* Contributing to Complex Wound and Leg Ulcer Clinics across the service.
* Coordinating care in collaboration with Community Nursing Network teams.
* Supporting patient self-care, long-term condition management, and health promotion initiatives.
* Contributing to innovative projects that enhance patient outcomes and service delivery.
What We’re Looking For
* A Registered Nurse with enthusiasm, motivation, and a keen interest in Wound Care.
* A commitment to delivering high standards of compassionate, patient-centred care.
* Excellent communication and interpersonal skills.
* The ability to work independently and as part of a multidisciplinary team.
* Adaptability, reliability, and a proactive approach to care delivery.
Essential Requirements
* A full driving licence and access to a vehicle, as the role involves travel across Kirklees to attend clinics and visit patients at home.
