As a Community Mental Health Practitioner, you will manage a caseload and provide a range of specialist mental health assessments and evidence-based, recovery-focused interventions. You will work collaboratively as part of a Multi-Disciplinary Team with individuals experiencing severe and enduring mental health difficulties and complex presentations, their carers, and other agencies, enabling access to mental health support.

Your responsibilities will include providing high standards of clinical care, ensuring safe and effective assessments, treatments, and discharge planning using clinical reasoning skills and appropriate assessment tools. You will manage your caseload and contribute to service and trust productivity requirements. Working with individuals, you will assess mental health needs, recognize mental illnesses, and help them understand, manage, or change their behavior.
You will plan, implement, review, and improve interventions to meet identified needs and manage inherent risks. Additionally, you will assess the needs of carers and families, developing and reviewing support programs. Maintaining accurate and timely clinical records, adhering to professional conduct, ethics, and legislation, and developing clinical practice in line with NICE guidelines and best practices are also essential.
