An exciting opportunity has arisen for a Community Care Assistant to join the Assessment and Review Team.
Successful applicants will be supported by a Team Manager and will work in collaboration with colleagues from other disciplines within the team which comprises Occupational Therapists, Social Workers, Community Care Coordinators and technology staff.
The team are responsible for the assessment for new community services, review of existing care at home services and oversight of unmet need in relation to Care at Home.
The successful applicant will have the opportunity to support clients to maximise independence whilst supporting the assessment and review workload.
The successful applicant must have the ability to work as part of a team, respond to the demands of a busy service, be able to follow procedures, have excellent communication and good report writing skills. They must have the ability to travel throughout West Lothian and hold a recognised qualification in social care.

The desirable skills and knowledge applicants could have are: -
SVQ level 3
e.g. HNC/SVQ level 3 in Social Care or equivalent
Experience in a SW Practice Team or OT Service
