Role Overview
The London Spinal Cord Injury Centre is looking for a dynamic person who is interested in promoting reintegration for newly injured patients. Applicants will need extensive experience working in the field of rehabilitation. The role involves working with patients on the ward, in therapies and in a community setting. The post holder will need to be flexible in their working hours to ensure patients are offered reintegration activities that accommodate their individual needs.
Key Responsibilities
* Participate in the care, rehabilitation, reintegration and discharge of patients with spinal cord injury and those with associated complex needs, in conjunction with other members of the Multi‑Disciplinary team.
* Promote and encourage social reintegration for spinal cord injury patients through sports, leisure and activities of daily living.
* Ensure continuity of appropriate care for spinal cord injury patients who are being discharged, through close liaison and handover of care needs to patients, families and relevant community professionals.
* Deliver appropriate education and discharge preparation to patients, their relatives and community professionals.
* Work independently with patients under the care of the LSCIC consultants on the ward or therapy environment, to deliver and evaluate their care and report back to qualified staff as appropriate.
* Deliver and evaluate care and assistance to SCIC patients both in the hospital and community settings (e.g., home, nursing homes, NHS facilities).
* Promote, encourage and provide the means for social reintegration to patients under the care of the Spinal Cord Injury Centre team.
* Liaise with SCIC staff and other members of the MDT regarding social reintegration and community inclusion activities; assist patients in organising appropriate sports, leisure and community activities.
* Maintain and renew the specific competencies required to provide autonomous care to patients who require mechanical ventilation.
* Deliver care to patients requiring mechanical ventilation both internally and externally, independently, using specific knowledge and adjusting care as the patient’s condition warrants and/or in emergency situations outside the ward environment.
* Demonstrate an in‑depth knowledge and understanding of long‑term effects and challenges facing those with a spinal cord injury.
* At the request of a Case Manager, assist in planning, implementing and delivering care to specific patients; evaluate the care and assist in the formulation of a community care plan with the Case Manager prior to discharge.
* Ensure all patients have the opportunity and means to participate in all aspects of social reintegration activities.
