Overview
A Practice Nurse plays a crucial role in delivering high-quality nursing care within a general practice setting.
The post holder will act within their professional boundaries to provide caring, compassionate holistic care for patients with a wide range of health needs. They will demonstrate safe, competent clinical decision-making and expert care, including assessment and diagnostic skills, for patients within the general practice. The post holder will work alongside GPs, healthcare assistants, and other primary care professionals to provide patient-centred care, health promotion, and disease prevention services.
Responsibilities
* The post holder will act within their professional boundaries to provide caring, compassionate holistic care for patients with a wide range of health needs. They will demonstrate safe, competent clinical decision-making and expert care, including assessment and diagnostic skills, for patients within the general practice
* Provide a FENO Testing service to appropriate patients across the PCN. Full Training for equipment will be provided
* Liaise with Respiratory Leads to improve the management and treatment of patients in this category
* Manage long-term conditions (e.g., asthma, COPD)
* Perform phlebotomy (if required)
* Liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients, etc.)
* Delegate clinical responsibilities appropriately (ensuring safe practice and that the task is within the scope of practice of the individual)
* Support the clinical team with all safeguarding matters in accordance with local and national policies
* Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately
* Health Promotion & Education: Offer lifestyle advice on smoking cessation, weight management, and exercise; Educate patients on chronic disease management and self-care strategies; Liaise with Health and Well Being Coach where appropriate; Support patients to adopt health promotion strategies that promote healthy lifestyles and apply principles of self-care
* Administration & Record-Keeping: Maintain accurate patient records using clinical software systems; Contribute to local and national practice targets clinical remit (e.g. QOF, prescribing incentive scheme, National benchmarking); Ensure compliance with infection control and health and safety policies; Participate in audit and the audit cycle; Monitor and ensure the safe storage, rotation and disposal of medicaments; Participate in local initiatives to enhance service delivery and patient care; Develop an area of specialist interest and lead within the organisation; Review and improve clinical practices in line with national policies and initiatives
* Health & Safety: Promote and maintain health, safety and security per practice policies; Identify risks and manage activities safely; Use infection control procedures; Report health and safety hazards promptly; Maintain cleanliness and safe work areas; Undertake periodic infection control training; Manage waste in accordance with policy

* Contribution to the implementation of services: Apply practice policies and guidance; Discuss policy impact with the team; Participate in audit where appropriate
Qualifications and Experience
* Essential
o Registered Nurse with the Nursing and Midwifery Council and maintains revalidation in line with NMC requirements
o Post graduate diploma or degree for Advanced Practice Qualification
o Meets NMC revalidation requirements in accordance with the NMC Revalidation booklet
o Holds more than one academic Level 6 diploma or postgraduate certification in long-term conditions care and/or public health initiatives
o Demonstrable experience in leading long-term conditions management and quality improvement within a PCN
o Working at Registered Nurse Level Practice as described in the Primary Care and General Practice Nursing Career and Core Capabilities Framework

o Additional training in chronic disease management or immunisations
* Experience
Essential: Experience as a practice or community nurse; working in a primary care environment; supporting quality improvement; involvement in immunisations, cervical screening; population health management; mentoring others; collaborating across PCN
* Desirable
Experience with Clinical systems (e.g., System One or EMIS); leading long-term condition management across the PCN
* Knowledge & Understanding: Experience in primary care/community settings; ability to work independently and in a team; competency in electronic patient record systems; knowledge of NHS, GP/PCN issues; clinical knowledge including hypertension, asthma, CHD; evidence-based practice; clinical governance; ability to refer to GPs; health promotion knowledge
* Personal Skills and Qualities: Essential: clear communication; organisation; record keeping; time management; composure under pressure; initiative; IT skills; commitment to quality and equal opportunities; teamwork. Desirable: integrity, empathy in distressing situations, ability to work under pressure
