An exciting 6 months secondment opportunity has arisen in the Pennine Care Community Rehabilitation Team for a motivated and enthusiastic Registered Mental Health Nurse.
The successful candidate will have significant experience and knowledge of working with people with chronic relapsing and remitting mental ill health, including experience of working across a number of services including secondary care and mental health inpatient services.
The team supports service users to transition from inpatient mental health rehabilitation facilities to living in the community, supporting people to improve their quality of life and work towards achieving the goals and outcomes identified by those who we support.
The team has a strong emphasis on the Recovery Model, working with service users using a strengths based and trauma informed approach to maximise their independence and autonomy to improve their quality of life.
You will work with a team of experienced practitioners with a strong biopsychosocial, evidence based interventions.
You will receive regular supervision and your personal development priorities will be shared and supported through our appraisal process.
The team is a trust wide service covering all five boroughs of Pennine Care and it is therefore essential that you drive and have access to a car. We are currently based at Trust HQ in Ashton Under Lyne. Our core hours are Monday to Friday 9am-5pm.
Main duties of the job
The practitioner will provide interventions to service users within the Community Rehabilitation Team who experience complex psychosis, often with other complexities such as substance misuse or physical health needs.
To work with the multidisciplinary team to offer a team approach to supporting service users either in the community or for those on the discharge pathway from inpatient settings.
To work collaboratively across a range of disciplines to engage service users in psychologically informed treatment plans.
To contribute to case formulation and treatment plans.
To contribute to service development in collaboration with the wider team.
To participate in the education programmes of nursing students and other disciplines.

Essential Criteria
* Registered Mental Health Nurse registration
* Experience of working in rehabilitation services
* Experience of working with people with severe and enduring mental illness
* Experience of working with people with behaviours that challenge and/or substance misuse problems
* Experience of providing support to vulnerable / socially excluded individuals/ groups
* Experience of providing supervision
* Extensive knowledge the recovery model and strength based principles
* Knowledge of mental health and social care national policy and legislation
* Effective risk assessment and risk management skills
* Ability to case manage a caseload of service users using rehabilitation principles
* Ability to develop trusting relationships with both service users and carers
