We are excited to announce an opportunity for a dedicated and compassionate Registered Nurse to join our expanding Frailty Service in the North Sector. This is a pivotal role in shaping the future of integrated care for older adults living with frailty.
The Frailty Service in the North Sector is a dynamic and evolving model of care designed to meet the complex needs of older adults living with frailty. Based at Glasgow Royal Infirmary, the service integrates early identification using the HIS Frailty Assessment Tool, rapid access to Comprehensive Geriatric Assessment (CGA), and coordinated multidisciplinary input. The service has proudly demonstrated measurable improvements in patient outcomes, reduced length of stay and enhanced discharge planning. The team works closely with health and social care partners to deliver person-centred care, and is committed to continuous improvement, education, and innovation in frailty management.
As an ECAN, you will be at the forefront of delivering high-quality, person-centred assessments for Older Adults. You will work closely with multidisciplinary teams to ensure timely interventions, reduce hospital admissions, and support safe discharge planning.
Key Responsibilities
* Support early identification of frailty and proactive management strategies.
* Liaise with GPs, hospital teams, social care, and voluntary sector partners
* Conduct comprehensive frailty assessments and contribute to personalised care planning
* Participate in service development and quality improvement initiatives
About You
* Registered Nurse with current NMC registration
* Experience in elderly care, community nursing, or frailty services
* Strong communication and assessment skills
* Ability to work autonomously and collaboratively
* Passionate about improving outcomes for older adults
