Medicines Management Pharmacy Technician Community and Mental Health
Band 5 Pharmacy Technician with an interest in supporting medicines optimisations within community hospitals, mental health, domiciliary visits and virtual wards. This post covers Weymouth & Portland, Jurassic and Mid Dorset areas.
Applications also accepted from Pre‑registration Trainee Pharmacy Technicians due to qualify in February 2026.
The role leads improvements to maximise safe, cost‑effective best practice in prescribing to improve patient care. The post holder helps patients get the best from their medicines by providing medicine optimisation in community hospitals or the domiciliary setting, and is part of the virtual ward MDT team.
The role may also require enrolment on the Pharmacy Medicines Optimisation Training programme and may offer opportunity to undertake ACT. Candidates who require current Skilled Worker sponsorship to work in the UK are not eligible.
Responsibilities
* Undertake patient facing and patient supporting roles.
* Undertake medicines optimisation in community hospitals, virtual ward and domiciliary setting.
* Support medication reviews and medicines reconciliation.
* Provide specialist expertise where competent.
* Support initiatives for antimicrobial stewardship.
* Work with the virtual ward MDT to ensure efficient medicines optimisation.
* Provide training / support on the legal, safe and secure handling of medicines.
* Develop relationships and work with practice teams and community pharmacies.
* Support the implementation of national prescribing policies and guidance.
* Undertake clinical audits, support quality improvement measures and contribute to the Quality and Outcomes Framework and enhanced services.
* Assist in the delivery of medicines optimisation incentive schemes and patient safety audits.
* Provide leadership for medicines management systems across all services.
Eligibility & Requirements
Band 5 Pharmacy Technician, or Pre‑registration Trainee Pharmacy Technician due to qualify in February 2026. Must be able to work in the UK without Skilled Worker sponsorship or apply for alternative routes. Willingness to enrol on the Pharmacy Medicines Optimisation Training programme and undertake ACT if required.
