Job Description
Liverpool City Council is seeking Reablement Assistants to join the Adult Social Care and Health Directorate within the Intermediate Care and Reablement service. Based at the Granby Hub, a 30-bed reablement unit, this role focuses on supporting individuals to regain and retain independence through personalised care and enablement.
Key Responsibilities
* Support service users with physical, social, and emotional needs as outlined in care plans.
* Assist individuals with daily living tasks, including kitchen skills, household maintenance, and personal care.
* Promote independence and dignity while respecting cultural, spiritual, and religious needs.
* Work alongside families, allied health professionals, and social care teams to deliver coordinated services.
* Administer medication where required (community-based staff).
* Contribute to assessments, reviews, and person-centred support plans.
* Encourage participation in recreational, social, and therapeutic activities.
* Maintain accurate records and report changes in circumstances to line managers.
* Uphold council policies, including equal opportunities and anti-discriminatory practice.
Requirements
Essential
* NVQ Level 3 in Health and Social Care or willingness to work towards certification.
* Experience in adult social care settings.
* Strong communication and listening skills.
* Ability to follow reablement programmes and relate effectively to others.
* Flexibility to work shifts between 8am and 10pm, across 7 days.
* Enhanced DBS check (Adult Workforce and Adult Barred List).
Desirable
* Certificate in Manual Handling.
* Experience delivering person-centred care.
* Teamworking skills and commitment to Liverpool City Council’s values.
