Overview
Applications are invited for a Full-Time (10PA) post of Consultant Geriatrician, with a Specialist Interest in Parkinson's Disease, at Bradford Teaching Hospitals NHS Foundation Trust. The successful candidate will work alongside an enthusiastic team of 14 Consultant Geriatricians, matrons, nurses, ANPs, supported by specialty registrars and FY1/2 doctors.
The elderly care department includes 4 acute wards, 4 community hospitals and a virtual ward. Intermediate care services include Community Hospitals and Virtual Ward, providing opportunities to develop a more effective interface between acute and community-based services for our older population with a discharge to access model.
Responsibilities
* Work with the existing team of consultants to deliver the Elderly Care service across the trust, with particular focus on interface geriatrics and developing community-based services with a focus on admission and readmission avoidance.
* Undertake a weekly Parkinson's disease clinic and work closely with Neurology colleagues to develop a robust Parkinson's disease service for frail older people in Bradford.
* Support the expansion of orthogeriatric services at the trust to include frail older people based on the orthopaedic ward with fractures other than hip fractures and involvement of geriatricians in the elective orthopaedic waiting list.
* Support Virtual Ward that has been established as an enabling multidisciplinary team (nursing, medical, therapy and social care) to support older people at home, including a monitored discharge process via a "step down" model and an ambulatory care pathway.
* Contribute to the development of an embedding of a comprehensive geriatric assessment in all patients to prevent admissions from primary care.
* Develop services aimed at avoiding readmissions to hospital through active follow up of high-risk patients on discharge from hospital.
* Take on significant responsibility in supporting activities for the department within areas of clinical governance, including service development, clinical governance, MDT review, clinical trials, mortality and business meetings.
* Undertake regular inpatient ward rounds and provide medical support on the Elderly assessment unit, focusing on admission avoidance and appropriate community-based care.
* Participate in the department's service development and collaborate with consultant colleagues on ongoing commitments and areas of interest.
