We have a peripatetic Social Worker opportunity within Adult Services, welcoming applications from both newly qualified and experienced social workers. You will work across all Adult Services teams, including Learning Disability, Mental Health, Integrated Discharge, Long-term Care, and Intake and Assessment, on agreed timescales. This role is ideal if you wish to develop your skills and experience in different teams within Adult Services.

About the role
Pay Details: Grade 8-9, (Depending on experience), SCP 26 – 35, £36,124 – £44,711 per annum
Hours of Work / Working Pattern: 37 hours per week, 8:30 – 5:00 pm (4:30 pm Friday) with some flexibility.
Main Place of Work: Varies
Contract Type: Permanent
Description:
* Undertake assessments of citizens in accordance with statutory duties under relevant legislation.
* Plan and commission services to enable adults with care and support needs and carers to overcome barriers, achieving personal outcomes, and monitor and review care plans through co-production with citizens and carers.
* Participate in supervision and Personal Development Review sessions, identifying personal development and training needs to maintain registration with Social Care Wales.
* Undertake training to support professional development, including becoming an Approved Mental Health Practitioner or other senior roles.
* Supervise or mentor staff or students as appropriate and contribute to training programs and team meetings.
* Report safeguarding concerns promptly to the appropriate officer.
About you
You will need:
* Experience working with adults and young people eligible for Social Services support.
* Assessment skills for adults and young people.
* Experience supporting adults transitioning from placements into the community.
* Promoting independence and fulfilling lives for adults.
* Experience working with statutory and voluntary organizations to promote independence.
* Knowledge of legislation such as the Social Services and Wellbeing (Wales) Act, Disability Discrimination Act, Mental Health Act, and Mental Capacity Act.
* Understanding of safeguarding procedures for adults.
* Commitment to Equal Opportunities policies.
* Effective communication skills, both verbal and written.
* Ability to assess needs, plan, prioritize, and review work.
* Analytical and evaluative skills.
* Negotiation skills with individuals, professionals, and agencies.
* Teamwork and flexibility skills.
