Job Description:
Active Care Group’s Care in the Home division continues to grow, providing safe, reliable, and outcome-based care through our support workers, clinicians, and management team.
Are you a clinician tired of long shifts and seeking better work-life balance?
We are expanding our team and seeking a Clinical Manager to join the Care in The Home division, employing Nurses as Clinical Managers.
This is an exciting opportunity to be part of a division that supports children, young people, and adults with conditions such as brain injury, spinal cord injury, neurological and neurodegenerative diseases, respiratory, and gastrointestinal conditions, including PEG feeding.
We seek someone with strong clinical skills, leadership abilities, and a passion for teaching and mentoring, working closely with a team to support individuals with complex needs to live safely at home.
Please note: We cannot sponsor candidates requiring a UK work visa for this role.
Work Schedule:
* Monday - Friday, with on-call duties for emergencies
* Home-based with regional travel (Chester, Llanfairfechan, Cyfflliog, Ruthin, Saughall, Flint, Liverpool, Preston)
Responsibilities:
* Provide competency-based training for clients with complex needs
* Delegate clinical skills to Support Workers and Care Managers after training and assessment
* Maintain and update Personal Care Plans in line with company protocols
* Conduct annual observations and ensure competencies are current
* Address safeguarding issues calmly and accurately
* Communicate with ICBs and Care Funders regularly
* Collaborate with scheduling and care teams about care risks
* Audit patient records and MAR charts monthly
* Visit clients at home to teach and assess competencies
* Ensure accurate documentation and support worker welfare
* Think strategically and see the bigger picture in complex care management
Qualifications:
* Current NMC Pin registration (Dual RN/RSCN or Adult Nurse with recent pediatric experience)
* Legal right to work in the UK
* Full driving license and own vehicle
* Knowledge of vents, bowel/bladder management, tracheostomy, catheterization, epilepsy, blood glucose monitoring, NIV, Nebs, PEG, challenging behavior, ADHD, and autism
