Job Title: Care Coordinator
Role Overview
We are seeking an organised, compassionate and proactive Care Coordinator to join our domiciliary care team. You will play a vital role in ensuring high-quality care is delivered to our service users by effectively scheduling care visits, supporting care staff, and maintaining strong communication with clients and families.
Key Responsibilities
* Coordinate and schedule care visits using care management systems
* Match carers to service users based on skills and preferences
* Handle rota planning, including sickness and emergency cover
* Communicate with clients, families and care staff professionally
* Support onboarding and training of care staff
* Monitor compliance with CQC standards and company policies
* Handle enquiries and concerns in a timely, empathetic manner
* Maintain accurate records and reports
* Take part in On Call duties
About You
* Previous experience in domiciliary care coordination (preferred)
* Strong organisational and time management skills
* Excellent communication and people skills
* Confident using scheduling and care management software
* Calm under pressure and able to manage competing priorities
* Knowledge of CQC regulations and care standards
* A compassionate and professional approach
* Must be a driver with a vehicle
