This is a great opportunity to join a supportive team providing rehabilitation under the guidance of clinicians to help people to become more independent and reach their personalised goals after a stroke.
We are looking for someone who is passionate about patient care and a good team worker. It is ideally suited to someone who has a background in care work looking to move more into rehabilitation or someone with experience in healthcare or rehab looking to move into a community role.
The role is based primarily in peoples homes and local community and could be providing personal care and meal preparation but also would be following care plans working on goals for problems with mobility, dexterity, cognition, vision, hand and arm movement or communication or may be to take clinical observations.
We have 2 positions available:
* Bristol - 37.5hours a week
* South Gloucestershire - 30 hours a week
Main duties of the job
To carry out rehabilitation plans as prescribed by therapists
Provide assistance to clinical staff in the assessment and treatment of patients
Provide assistance with activities of daily living as per care plan
Be involved in stockchecking of equipment and some basic administration jobs
Attending meetings and training as needed
To escalate concerns about therapy programmes or deteriorating patients to senior colleagues
To work as part of a multidisciplinary team
Qualifications and Experience
Experience of working with adults in a health and social care setting
Able to work without direct supervision
Able to work as part of a team
Able to competently use IT (emails, word documents, excel)
Able to prioritise tasks
Experience of working in community setting
Experience of community rehab or stroke rehab
