An exciting opportunity has arisen for an experienced Nurse or dietitian to take on a clinical and professional leadership role within the community diabetes service at Guys and St Thomas NHS Trust.

We are seeking a dynamic individual with proven experience in a senior clinical leadership role, who can demonstrate a track record of enhancing both patient and staff experience within a complex healthcare environment. If you are passionate about advancing community Diabetes services and leading the professional and clinical development of nursing, dietitians and pharmacists, we invite you to join us.

Your ability to drive service improvements while maintaining a strong focus on patient-centred care is essential. Strong communication skills and the ability to work effectively across multiple locations and multi-professional boundaries is key.

Main duties of the job

1. Provide clinical leadership and supervision to the community diabetes Team
2. Clinically lead community diabetes care throughout the various patient pathways
3. Provide education and support to community diabetes team consisting of doctors, pharmacists, nurses and dietitians.
4. Provide education to the multidisciplinary teams, responsible for the education pathway of staff
5. Enable collaboration across the multi-disciplinary teams through complex decision making
6. Manage risk thus enabling the services to meet changing population,
patient, and service delivery needs.
7. Transformation, quality and improvement initiatives to develop the services.

Detailed job description and main responsibilities

We are looking for a motivated, ambitious, and flexible nurse or dietician with experience in community settings. The successful candidate will provide clinical leadership to the community diabetes team and throughout the patient pathway and collaborate with the multidisciplinary team on service transformation, quality improvement, and innovation initiatives. You will also run nurse/ dietitian -led clinics and actively contribute to multidisciplinary clinics, audits, and operational meetings. Opportunities to participate in teaching and research are strongly encouraged.

Applicants must have experience in leading clinical services, with a proven commitment to prioritising safety, quality, and patient experience in all aspects of care.

11. Excellent inter personal and communication skills

Desirable criteria

12. Advanced clinical assessment and history taking
13. Evidence of Advanced theoretical and practical knowledge of diabetes management

previous experience

Essential criteria

14. Ability to lead and work within a community service with Nursing, medical, dietitians and pharmacists
15. Experience in writing/ reviewing clinical policy's/ guidelines SOP's and working with the MDT.
16. Leading on governance with experience in managing incidences, complaints and audit at a local and trust level
17. Experience of supervising and developing Junior nursing, dietetic or pharmacy staff
18. Demonstrates previous Experience in leading and delivering change at either a local or national level

Desirable criteria

19. Advanced Clinical Experience in diabetes management

Qualifications

Essential criteria

20. A Masters level qualification/in progress or equivalent experience
21. Non-medical prescriber independent or supplementary
