What the Job Involves You will support clients in achieving their reablement goals, working in collaboration with health professionals. Your duties will be guided by a service delivery plan and allocated via a mobile device. The role involves working shifts according to a rota, including potential evening and weekend work. You will help facilitate independent living for service users by preventing unnecessary hospital admissions and assisting with hospital discharges.
