In this key position within the Office of the Medical Director, you will play a central role in supporting the Patient Safety Incident Response Framework (PSIRF). Collaborating closely with clinical and governance teams you will provide leadership and oversight for all PSIRF activities, identifying opportunities for learning and improvement driving high-quality, person-centred care.

This position is a 12-month fixed-term role to fill an important staffing gap in the directorate.
Main duties of the job
* The post holder oversees the daily management of patient safety activities overseeing the most complex investigations, supporting the complex case investigators and working with divisions to investigate PSIIs.
* The postholder has a defined caseload of complex investigations and learning responses, including those involving complaints, claims, and inquests.
* The post holder conducts comprehensive data analysis of all incidents, generating reports on emerging themes and collaborating with divisions and the safety improvement team to identify and implement learning and improvement opportunities.
* They will take the lead for agreed learning responses, facilitate After Action Reviews (AARs), and conduct Patient Safety Incident Investigations (PSIIs) & MDT reviews in partnership with the appointed learning response leads.
* The post holder ensures the governance framework for the Patient Safety Incident Response Framework (PSIRF) is followed at all levels of the trust, providing assurance through weekly and monthly analysis and reports, and escalating any concerns or delays as needed.
* They will ensure all PSIRF processes are fully understood and embedded, offering expert advice to divisions. Once learning responses or investigations are determined, they ensure all statutory requirements are fulfilled and that final reports meet the needs of all stakeholders involved.

Person Specification
Education/ Qualifications
* Educated to Masters level or equivalent
* Vocational qualification in health or management related subject
* Evidence of continuing professional development
Experience
* Clinical background or evidence of a substantial understanding of clinical issues
* Significant experience of undertaking and managing serious incident investigations
* Experience of working as a senior manager in the NHS
* In-depth knowledge of healthcare risk management policy in the NHS; NHS risk management principles; and NHS (Datix) incident reporting system
* Substantial experience of clinical governance
* Evidence of experience in serious incident investigation report writing
* Ability to design and deliver effective training programmes
* Understanding of disease processes and terminology
Skills
* Skilled in data analysis and interpretation
* Experience of providing Management and Board level reports.
* High level of presentational skills.
* Proficient computer literacy skills and experience of using computer databases and spread sheet applications
