Overview

The Urgent Community Response (UCR) Team is recruiting one full-time Clinical Assessor to be based at Parkhouse in Gedling. This role is based across the whole of South Nottinghamshire (Gedling, Broxtowe and Rushcliffe). UCR provides holistic, person-centred care with the aim of preventing unnecessary hospital admissions and enabling individuals to increase independence and improve health outcomes.

Urgent Community Response (UCR) collaborates with EMAS, primary care, social care and community health partners, as well as care and lifeline providers, to deliver timely assessment, interventions, rehabilitation, management of long-term and life-limiting conditions, and partnership working across health and social care communities. The service operates 7 days a week from 08:00 to 22:00.

UCR is an NHS England national service aiming to review patients in their usual place of residence within 2 hours to reduce the risk of potentially avoidable admission. The team is collaborative, supportive, and focused on developing clinical reasoning and practical skills to enhance careers.

You will work with a multi-disciplinary clinical team and coordinate with local hospitals, EMAS, NHS 111, GP practices, community nursing and therapy colleagues, Local Authorities and private providers to enable recovery and discharge, and to prevent hospital admission.


Responsibilities

* Analyse and develop treatment plans within a multidisciplinary framework, providing compassionate, timely support to a diverse patient group and their families.
* Undertake holistic assessments of health and social care needs and perform comprehensive physical health checks; provide urgent equipment and safety netting advice.
* Prioritise factors in the patient’s presenting condition using clinical reasoning and knowledge of prognosis and related issues.
* Manage a caseload of patients with moderate to complex needs, with supervision from more senior clinical staff; support rehabilitation and reablement at home to promote independence and reduce future admissions.
* Demonstrate excellent communication and interpersonal skills; build effective working relationships with patients, staff and key holders internal and external to the Trust.


Qualifications and Requirements

* Registered Nurse (RGN) or RN (Adult) or Allied Health Professional with demonstrable clinical experience and evidence of continuing professional development.
* Full UK driving licence and vehicle for business use (reasonable adjustments available under the Equality Act 2010).

* Based at Park House in Gedling; also covering Cotgrave (Rushcliffe) and Stapleford Care Centre (Broxtowe).
* Ideal for nurses, physiotherapists, occupational therapists and other AHPs to join a new and innovative service focusing on development, innovation and compassionate care.
* Accountable and autonomous practitioner working with integrated care teams and acute Trusts to support discharge and prevent hospital admission.

DBS and Application
