The Community Rehabilitation Service provides a flexible range of mental health interventions to help clients with complex mental health needs manage their mental health more effectively and remain in their own homes. The team works in close partnership with Social Care and housing providers to ensure each client has the necessary resources available to enable them to engage in their rehab journey. Across the Community Rehabilitation teams, the clinical team consists of Occupational Therapists, Psychologists, Nurses, and VCSE Workers. We currently have an exciting opportunity for two enthusiastic and motivated Healthcare Support Workers to join our West Kent Community Rehabilitation Service. Post one will cover Maidstone (base will be Albion Place). Post two will cover South West Kent (base will be Highlands House). Individuals may be expected to ensure the team is able to provide input between the hours of 08:00 and 20:00 according to the needs of the service, including working some evenings, weekends and public holidays.
Responsibilities
* Work as part of a Community Rehabilitation Team, focusing on the direct needs of service users.
* Under guidance and supervision of the Multi-Disciplinary Team, provide support and give time to an allocated group of service users to promote their recovery and maintain them in their community environment, promoting and facilitating therapeutic activity and supporting self‑care and activities of daily living.
* Support service users to identify their needs and assist clinicians to plan, implement and evaluate individual care plans.
* Work to the recovery model to support the service user in achieving their identified goals.
* Give physical health checks and physical health care as required or instructed, documenting the checks on the early warning scoring system (MEWS), charting and reporting changes to the nurse/healthcare professional in charge.
* Undertake day‑to‑day tasks as allocated by nurse/healthcare professional.
* Be available between 08:00 and 20:00, including some evenings, weekends and public holidays.
Qualifications & Requirements
* Full, valid UK driving licence and vehicle available for business purposes (unless disability defined by the Equality Act 2010).
* Interest in supporting individuals with mental health needs.
* Good communication skills and ability to work collaboratively with a multidisciplinary team.
* Compassionate, supportive and committed to a recovery‑oriented approach.
