The post holder will be responsible for supporting the achievement of the 18-week referral to treatment target and the national cancer targets, including taking appropriate actions to ensure that the trusttargets are being met for the Specialty Business Unit. The post holderwill assistintheeffectivecollectionandrecordingofthepatientpathwaysacrossrenal and will support all administrative and clerical issues relating to patients’ pathway of care, trackingandsupportingthemfromreferraltotreatmentand/ordischarge.Thiswillinclude discussions with key members of the team as well as the Trust performance team to achieve patient carewithin target timescales.The post holder will be expected to produce weekly reports/updates forinternal meetings on the 18 week and cancer target position. The post holder is expected to actively monitor the 18-week risks, the cancer charts – including tertiary patients, any over 18-week patients, giving regular updatesof actions being taken.
Main duties of the job
The post holder will develop and maintain effective working relationships with clinicians,managersandotherswithinRenal and Trust wide service users and workacrossthe Specialty BusinessUnitandTrustandcontributeto the delivery of performance targets for services as appropriate. Duties will include:
Providing akeylinkbetweenthedepartment’smanagement team ensuringthat National 18-week Referral to Treatment (RTT)standardsandthenationalcancertargetstandardand any other local target set by the Trust are managed/coordinatedeffectivelyanddeadlinesaremet.
Undertakeinformationcollectionandanalysistosupportthe 18-week RTT pathways, escalating accordingly at performance meetings
Effectively support Renal in maintaining anopenandhonestculturethatfocussesonthedevelopment of staff and enables individual and team responsibility
Bethepointofcontactforallroutineenquiresfacilitating communication andinformation flows, initiating, and respondingtocorrespondenceandresolvingenquiries.
Engagewithsecretariesandconsultantsweeklyandleadon PTL meetings to identify pathways that can be closed.

The ICB includes 4 places (previously referred to as acute trusts) with 6 renal centres in addition to our main hub unit at Royal Preston Hospital.

ActivelyseeksinformationfromvariousIT systemssuchasEvolveFLEXQlikview. Use this information to identify any problems/risksinthepathwaysandensure that theseare resolved.
Addressperformanceissuesandescalate to management where necessary
Using their own initiative, supported by local guidance and the Access Policy, to implement, analyse, interpret and apply judgements involving the 18-week rules and definitions to ensure compliance
Responsible for ensuring that patient records have the correct clock starts and clock stops, by validating and interpreting patient pathways from first outpatient appointment, through diagnostics, treatment planning and treatment. This may involve adjusting / correcting patient referrals where inaccurate data entries have been put in by divisions.
Monitor the PTLs to maintain accurate waiting times throughout the patient pathway.
Contributetothecollectionofdataforaudit purposesandsupportmanagementteams to identify areas thatrequire pathway improvement.
Assistinscheduledandadhocauditsof processes of work,highlightingareaswheretarget achievementcouldbecompromised.
Monitorandassistwiththeimplementation of changesto recording of activity to ensure delivery of income.
Keepmanagersupdatedonwork, updating timelines,providing reports/updatesoncurrentposition, escalating when necessary.
Toprovidecoverforcolleaguesduring periods ofplanned annual leave and sickness

Qualifications & Education
Essential criteria
1. GCSE English and Mathematics Grade C or Above OR Equivalent
Knowledge & Experience

2. Experience of working/dealing with others
3. Experience of working in a busy NHS environment
4. A good understanding of clinical services within a hospital setting.
5. Awareness/understanding of 18week RTT Target
6. Awareness/understanding of cancer targets
7. Knowledge of patient pathway from referral to discharge
Desirable criteria
8. In depth knowledge of renal services and pathways
9. In-depth knowledge current breach tolerances and renal position
10. Proven experience in successfully managing both National and Local KPI’s and escalating as needed
Skills & Abilities
Essential criteria
11. Experience of monitoring and auditing processes of work.
12. A sound understanding and operational experience of specialist clinical systems i.e. Quadramed and ALMA
13. Able to grasp new concepts quickly and produce timely written reports at short notice
Desirable criteria
14. A sound understanding and operational experience of specialist clinical systems i.e. VitalPulse/Vitaldata; TMON; IRIMs.
15. An understanding of renal as a tertiary service and the clinics and renal centres we operate from and within.
16. An understanding of renal pathways.
