* Brook Lane Surgery is seeking a motivated and compassionate General Practice Nurse (GPN) to deliver high quality, evidence based nursing care to our registered patient population.
* The post holder will work both autonomously and collaboratively within the team. The role focuses on long term condition management, preventative healthcare, immunisations, and treatment room services, contributing to improved health outcomes and patient experience.
Main duties of the job
Deliver a full range of treatment room services, including:
* Wound assessment, care and dressings
* Removal of sutures and staples
* Leg ulcer assessment and management
* Phlebotomy and administration of injections
Undertake longterm condition reviews, within level of competence, including:
* Asthma and Chronic Obstructive Pulmonary Disease (COPD)
* Hypertension and cardiovascular disease
* Diabetes care
Administer immunisations and vaccinations (childhood, adult and seasonal) in line with national and local guidance
Carry out cervical screening in accordance with the NHS Cervical Screening Programme
Provide contraception advice and sexual health support within scope of practice
Identify, act upon and appropriately escalae clinical concerns or red flags to GPs or other clinicians
Support patients to improve health and wellbeing through lifestyle advice, including:
* Weight management
* Alcohol awareness
* Physical activity
Encourage and support selfmanagement for patients with longterm conditions
Ensure safe storage, stock control and cold chain compliance for vaccines and medicines
Maintain accurate, contemporaneous and confidential clinical records using the practices clinical system
* Quality and Outcomes Framework (QOF) and enhanced services
* Infection Prevention and Control standards
* Clinical audit and quality improvement activity

Qualifications
* Registered Nurse (Adult) with current NMC registration
* Experience working in general practice, primary care or a transferable community role
* Competence in core treatment room skills
* Ability to work independently, prioritise workload and demonstrate sound clinical judgement
* Excellent communication and interpersonal skills
* Good understanding of confidentiality, safeguarding and infection prevention and control
* Recognised Practice Nursing qualification
* Experience in chronic disease management
* Trained and accredited in cervical cytology
* Familiarity with GP clinical systems (e.g. EMIS)
