Overview
An exciting opportunity has arisen to join Lancashire South Cumbria Foundation Trust's (LSCFT) Central + West Network to work as a Senior Therapist. The role sits within the acute therapy delivery within Care Connexions provided by LSCFT as part of the Urgent Community Response, Virtual Ward and Home First teams, made up of physiotherapists, occupational therapists and technical instructors. The focus is on admission avoidance work in the community and early supported discharge from the acute hospital by providing a therapy assessment of patients function and care needs in their home environment following the principles of discharge to assess model.
The teams work collaboratively with other social care providers to provide an integrated health and social care team working together to ensure the most appropriate intermediate care services are commissioned based on the patient\'s needs. You will improve experience for patients and their families in relation to the discharge process; promote continuity of care from the acute into the community by supporting an integrated approach. Enable patient\'s to recover within their home environment after assessment of needs within their usual place of residence.
Responsibilities
* In this role there may be times where you are asked to cover across the wider community network including triage work as part of Care Connexions single point of access.
* Within the Specialist Therapist position part of the role will be to undertake clinical triage supporting timely discharges from acute ensuring the most appropriate services are commissioned and also to support admission avoidance triage.
* Collaborate with the Care Connexions team and social care providers to ensure integrated health and social care planning.
* Undertake therapy assessments of patients\' function and care needs in the home environment following discharge to assess model.
Requirements / Qualifications
* We are looking to appoint dynamic, enthusiastic and forward thinking therapist to join our service. With excellent communication skills who can demonstrate sound assessment skills and has the ability to work autonomously as well as part of a team.
* This post is subject to an enhanced DBS Disclosure.
* Part time hours will be considered for this post.
* Hours may be required to be worked over 7 days between 08:00 and 20:00, depending on service need.
