Main duties of the job
The post holder will be a key member of the senior nursing team, assisting with the maintenance of structures, systems, and processes to effectively coordinate admission avoidance and discharge activities across Medway NHS Foundation Trust within an integrated approach.
The role involves liaising with other intermediate care services within Medway, primarily community rehab teams, as well as Council colleagues.
Work with multidisciplinary teams to facilitate timely and appropriate clinical management of patients.
Develop and maintain good working relationships with inpatient and community services within the organization and with external bodies.
Participate actively in daily Board Rounds, contributing to the safe and timely discharge of patients.

Qualifications
* Registered General Nurse at degree level
* Registered mentor
* Degree modules specific to discharge, transfer of care, long-term conditions, emergency care
* Nurse prescriber
Experience
* At least 2 years as a registered nurse
* Previous experience in A&E or community nursing
Special knowledge / expertise
* Knowledge of national policy drivers and quality standards related to discharge and ED redesigns
* Awareness of ECIP publications, DTOCC

* Willingness to work occasional unsocial hours

Disclosure and Barring Service Check
This role requires a DBS check under the Rehabilitation of Offenders Act (Exceptions Order) 1975. A disclosure submission will be necessary to check for any criminal convictions.
Salary depends on experience.
