As the Substance Misuse Social Worker, you will: Work with adults whose primary social care needs are impacted by substance use Provide a duty/front door service offering advice, signposting, screening, and risk assessments Complete Care Act assessments and develop strength-based support plans Undertake initial care reviews and manage complex cases Fulfil statutory safeguarding duties, including Section 42 enquiries Collaborate with health professionals, housing services, voluntary sector, and other key partners Essential requirements of this Substance Misuse Social Worker rol...
