Post: Support and Outreach Worker X2

Salary: £24,251.92 – £26,442.94 per annum

Hours and Contract Type: 37 Hours, Full Time, Permanent

Responsible to: Support Team Manager

Department: Support Services

Location: Aberystwyth

Interview Date: 26 June 2025 and 4 August 2025

The main objectives of the post will be:

To work as part of a team of Support Workers across north and mid Ceredigion, under the direction of the Support Team Manager.

To deliver support through bespoke packages of care (not personal care) to service users who are vulnerable due to circumstances or conditions, including learning disabilities, mental health issues, autism, physical and mobility issues, etc.

Support for each individual is based on recognised support needs, providing practical help and assistance over time to improve quality of life, promote safety and emotional well-being, encourage independent living, companionship, and positive social integration into the community.

Role:

To work under the direction of the Support Team Manager and:

* Follow personal care plans, ensuring actions are agreed upon with the service user.
* Write, sign, and file support and outreach notes after each session, maintaining complete support files.
* Collaborate effectively with partner agencies for coordinated service delivery.
* Report safeguarding concerns to the Support Team Manager and liaise with statutory services as needed.
* Maintain electronic and paper-based records systematically.
* Manage personal mileage and expenses monthly, with oversight from the Support Team Manager and HR.
* Liaise with statutory and voluntary organisations (e.g., DSS, Local Authority, Social Services) for the benefit of service users and families.
* Participate in training, supervisions, and appraisals.
* Undertake additional duties as required.

* Attend team and organizational meetings, including training.
* Contribute to project development and promotion.
