The Care Coordinator will work collaboratively with administrators, GPs and other Primary Care Network (PCN) professionals to ensure patient care needs are met effectively and efficiently.
Main duties of the job
Working within a multidisciplinary team (MDT) model, the post-holder will support patients, families and carers, coordinating care across health, social and voluntary services where appropriate. The role includes patient care coordination, health and wellbeing promotion, administrative support and contribution to service improvement and quality.
The post-holder will support efforts to address health inequalities, contributing to improving access and outcomes for a diverse patient population and promote partnership working. The Care Coordinator will also provide operational support to ensure clinical provision is utilised effectively and that patients are supported through duty systems as required.
The scope of the role will be agreed with the individual Care Coordinator, taking account of their knowledge, skills, experience and completed competencies.

Qualifications
* Good standard of general education, including English and math.
Experience
* Experience within a healthcare setting.
* Blood pressure taking.
* Urinalysis
* venepuncture
* New patient health checks
* Previous Primary Care experience.
* Injections in line with PGS's/PSD's
* ECG's
* Simple wound care
* Ear syringing
* Experience of supporting people/families/carers in a similar role
* Warfarin INR monitoring in house and in community
* Assisting in minor operations
* Assisting in coil clinics
