Acute Liaison Team Manager: Learning Disabilities and Autism

South Tyneside and Sunderland Foundation Trust are looking to appoint an Acute Learning Disabilities and Autism Team Manager. You will lead a team that ensures adults with learning disabilities receive the right support to live independently and with dignity while accessing STSFT acute services.
You will be a member of the adult acute disability service but work closely with community learning disabilities teams supporting individuals with learning disabilities and/or autism across various settings within the trust.
The Learning Disability (LD) and Autism Liaison Services operate from 09:00 to 17:00, Monday to Friday, with occasional support outside these hours by prior arrangement in collaboration with community teams. We aim to contact all adult patients with LD and/or Autism admitted to hospital within 24 hours, working with patients, carers, ward staff, and community staff to ensure comparable healthcare outcomes for all.

Main duties of the job
The Post Holder will:
1. Act as a positive role model in LD nursing by demonstrating competent, accountable, and up-to-date clinical practice.
2. Lead the LD&A acute services to ensure compliance with training, legislation, policies, and procedures.
3. Support and line manage the acute team supporting wards and departments across the Trust.
4. Contribute to risk assessments and care plans to meet the needs of vulnerable individuals, providing advice on assessments such as mental capacity, safeguarding, best interest decisions, and deprivation of liberties.
5. Assess and advise on complex needs related to emergency and elective admissions, discharge planning, and access to investigations.
6. Provide specialist advice on managing the interface between hospital departments, community services, adult social care, and primary care, especially in complex cases.
7. Lead and manage mortality and LeDeR reviews in collaboration with LD&A community teams, ensuring learning is embedded.
8. Lead community-led delivery of mandated LD&A training.

Job responsibilities
An acute liaison team manager for learning disability and autism is a qualified professional responsible for leading an adult service and team, working collaboratively with community teams to provide expert care, guidance, and support based on advanced knowledge of specific conditions.
You will be accountable to the Assistant Director of Safeguarding for managing and developing the team, deploying staff, and ensuring service coordination and resource optimization.
You will maintain a caseload, contribute to care pathway planning, support clinical practice development, and provide expert advice on patient management. You will ensure standards and targets are met, lead on complex treatment regimes, facilitate hospital discharges, and prevent avoidable admissions.
You will exercise accountability as per the Professional Regulatory Code, line manage the team, and be responsible for clinical governance, quality monitoring, research, and audits. You will develop clinical expertise within the team, work autonomously, and act as a resource for multidisciplinary teams. You will also lead training, mortality reviews, and ensure adherence to the Trust’s values and policies.
