Family Support Worker
To provide high-quality social work and family support services to children and young people with disabilities and their families, ensuring that they are safeguarded, supported, and empowered to achieve their full potential.
The post holder will work collaboratively with families, professionals, and partner agencies to deliver child-centred, outcome-focused interventions that promote inclusion, independence, and resilience within the community.
Key Responsibilities as a Family Support Worker
Assessment & Planning
Undertake statutory assessments under the Children Act 1989 and the Care Act 2014, ensuring timely, proportionate, and evidence-based decision-making.
Develop, implement, and review Child in Need, Child Protection, and Looked After Children plans where required.
Contribute to Education, Health, and Care (EHC) planning for children with complex needs.
Direct Support to Families
Provide practical and emotional support to children, young people, and their families to strengthen family relationships and promote positive outcomes.
Empower parents and carers to develop skills, routines, and strategies to manage challenges associated with their child's disability.
Advocate for children and families to ensure their voices are heard and their rights are respected.
Safeguarding
Identify and manage risks to ensure the safety and wellbeing of children, escalating concerns in line with Sefton Council's safeguarding procedures.
Work effectively with the Multi-Agency Safeguarding Hub (MASH) and other statutory partners to protect children at risk of harm.
Multi-Agency Working
Build strong partnerships with education, health, voluntary, and community sector colleagues to deliver coordinated, holistic support.
Attend and contribute to multi-agency meetings, case conferences, and reviews.
Recording & Reporting
Maintain accurate, timely, and comprehensive case records using Sefton Council's electronic systems.
Prepare high-quality reports, assessments, and care plans for internal and external purposes, including court proceedings if required.
