An exciting opportunity has arisen within the Mid & Upper Nithsdale Home Team for an enthusiastic and passionate Band 6 Specialist Practitioner Community Nurse to join our Home Team. We deliver quality nursing care to housebound patients 24hrs per day.

This role leads and supports a well-established community nursing team covering a rural area, working closely with the Home Team Lead and Professional Leads across Thornhill, Cairn Valley and Sanquhar GP practice areas.

As part of this role, you will be working within an ever changing environment, alongside colleagues in the multidisciplinary Home Team, including Allied Health Professionals, Social Services & Public Health Improvement. The Home Team is integral to our Health and Social Care Partnership.

This is a highly skilled role and applicants will ideally have the minimum post-graduate qualifications to fulfil the requirements of this role, namely V300 independent/supplementary prescriber and advanced clinical assessment. If applying as a Band 5 there is an expectation that the individual will be able to show evidence of additional learning, and will be supported to complete the PG Certificate Integrated Community Nursing.

It is desirable that you have some experience in leading/ managing a team and also in caseload management. You must be a good communicator with excellent interpersonal skills and demonstrate an ability to work with different professional groups. It is desirable that the successful applicant be able to demonstrate an interest in quality improvement work and change management. You should also have a good awareness of national policies and a willingness to take an active lead in the implementation of these policies locally.

Experience of working within a community setting is preferred but is not essential.

This post will involve clinic and home visits in remote and rural areas. A current driving licence is an essential requirement of this post. However, if you have a disability which precludes you from holding a driving licence, consideration will be given if alternative transport can be arranged. Please detail how you would facilitate travel to clinics and home visits in your application.


As from 1st April 2026, the Agenda for Change full-time working week will be reducing from 37 to 36 hours per week. Part time hours will be reduced pro-rata. However, there will be a corresponding increase in the hourly rate so that pay will not be affected.
