Bolton are looking for a Social Worker for the Neighbourhood team, any adult aged 18+ with needs other than Learning Disability. The candidate will focus on outdated reviews, approximately 4 per week. Candidates with ongoing reviews experience in another authority are ideal.
The candidate will work with health professionals such as district nurses and therapists.
Required experience and knowledge include:
* Care Act assessment
* Strength-based support planning
* BIA and capacity reviews
* Safeguarding
Candidates must have at least 2 years experience within a specific neighbourhood team; otherwise, 3+ years is required.
Candidates must be local to Bolton and able to work in Bolton, with hybrid working (office and home). The role is based in Bolton, Bury, Wigan, and requires availability for short-notice visits.
Initially, candidates will work fully office-based for the first 4 weeks. The role involves collaboration with health colleagues to support adults and their carers under the Care Act 2014.
Bolton's teams include Social Workers/Assessors, Occupational Therapists, Nurses, and Mental Health Practitioners, focusing on person-centred, strength-based practices to provide high-quality care, protect vulnerable individuals, and foster supportive communities.
Main duties include completing Care Act assessments, support plans, reviews, Mental Capacity Assessments, and participating in Best Interests meetings. The role also involves working with individuals with complex needs, safeguarding, and legal issues, including investigations and enquiries.
The role utilizes resources from social care, health, voluntary, and statutory agencies, assessing individuals' independence and support needs on short- and long-term bases.
Administrative duties include case documentation, attending case conferences, reviews, and representing the Council in court when necessary.
