Beluga Rox is recruiting a Care Assessor for their client to support the delivery of safe, person-centred care across Chester in a role that’s varied, meaningful, and truly people-focused.
The Role
You’ll complete assessments and reviews, develop person-centred care plans, and keep accurate records using digital systems. You’ll also support care staff and liaise with families and healthcare professionals to ensure clients receive the right care at the right time.
Key Responsibilities
- Carry out new client assessments and regular care reviews
- Create and update care plans that reflect individual needs
- Maintain accurate records using digital care systems
- Support carers and communicate effectively with clients, families and professionals
- Take part in the on-call rota and provide emergency support when required
What We’re Looking For
- NVQ Level 3 in Health & Social Care (or above)
- Experience in domiciliary, community, or complex care
- Strong communication and organisational skills
- Full UK driving licence and access to your own vehicle
- £33,000 per annum
