What you’ll do
1. Lead, supervise, and motivate a team of support workers
2. Provide regular supervision and model best practice in complex cases
3. Oversee assessments, support plans, and reviews to ensure quality care
4. Maintain accurate records and monitor performance using digital systems
5. Work with external agencies to assess referrals and prioritise support
6. Build strong relationships with local services and professionals
