The Care Coordinator plays a key role in ensuring the highest quality care services:
1. Coordinate Care: Manage referrals, create care worker rotas, liaise with professionals (e.g., social workers, GPs), and monitor service users' conditions.

Experience & Organisation: You are an efficient, organized Care Coordinator passionate about community care. You excel in managing rotas to impeccable standards and pride yourself on building strong relationships with care workers.
