Do you fancy working in the Discharge to Assess team, where you would work with Health and social care colleagues and care homes, attending Continuing Health Care meetings and planning discharge from Discharge to Assess beds. We are a small county‑wide team dedicated to supporting customers and their carers in a fast‑paced environment.
About the role
By joining our established Discharge to Assess Team, you'll provide a social work service to a highly professional standard. You'll manage a caseload and travel throughout North and South Warwickshire, working with adults, their families and unpaid carers, and occasionally with people from acute hospital settings.
Key responsibilities include attending Continuing Health Care/Decision Support Tool (DST) meetings, planning discharge, undertaking robust assessments and care and support planning, communicating with a wide variety of customers, communities, health partners and agencies, and working on your own initiative in pressured situations.
Team members receive regular supervision and day‑to‑day support from the Team Manager, and have access to regular training provided by Warwickshire County Council.
Key Requirements
* Professional Social Work qualification and registration with the Health and Care Professions Council.
* Ability to work 5 days over 7 days, including weekends (one weekend per month).
* Willingness to work closely with the multidisciplinary Health & Social Care team and participate fully in team meetings and other meetings determined by the Team Manager and/or Service Manager.
* Driver’s licence essential for completing post‑discharge reviews, particularly in care homes.
* Dynamic approach, embrace a strengths‑based approach, and ability to make difficult decisions within tight deadlines.
* Experience in adult social care, good IT skills, and familiarity with the Mosaic IT system.
* Experience attending Continuing Health Care/Decision Support Tool (DST) meetings desirable; willingness to learn within a fast‑paced environment.
